• An official transcript from home university (all prior to the exchange).
• A photocopy of passport.
• A passport-sized photo (for Student ID Card)
• A study plan & Statement of purpose.
• Language Proficiency Certificate (if any, not required)

Please prepare ORIGINAL COPIES of completed Health Certificate (Valid for 3 months before the entry date) after receeiving your Letter of Admission. PLEASE NOTE THAT SOME EMBASSIS ALSO REQUIRE YOUR HEALTH CERTIFICATES BEFORE ISSUING THE VISA.

• If your stay in TW will NOT exceed 180 days: Health Certificate for Short-Term Students(Form C). Please print the form and bring it to your local clinic or hospital for stamps/doctor's signatures.
• If your stay in TW will be OVER 180 days: Health Certificate for Resident Application. Please print the form and bring it to your local clinic or hospital for stamps/doctor's signatures.
• Required Documents: the documents will need to be handed in on Orientation. You will be asked to do the health examination in Taiwan if you fail to provide the certificate.          2. Copy of test results/vaccine records.

Accident Insurance

• Accident insurance is MANDATORY for all incoming exchange students. Students are requested to join and purchase the accident insurance plan in addition to their own insurance from their home country/university offered by SU.
• Insurance Fee: NT$ 1,020 for 6 months (NT$170 per month)/ NT$2,040 for 12 months (two semesters) and will be collected upon orientation.

Health Insurance (OPTIONAL)

• According to the Central Epidemic Control Center (CECC) announcement, SU is now strongly regulating every exchange students to get health insurance with SU. If you already get health insurance in home country, you need to provide a copy of the insurance policy which proves that you did buy medical insurance to SU and sign the Affidavit. We'll have the students sign the Affidavit on the Orientation Day.

• Insurance Fee: NT 500 per month, the content of insurance is in the attachment. [NT 3,000 for 6 months (1 semester) / NT 6,000 for 12 months (2 semesters)]

The fee is subject to change according to the Group Insurance Package and contract every semester.
